Hospice RN Case Manager

This position involves providing specialized end-of-life care, supporting patients and their families through comprehensive nursing services. The RN Case Manager assesses patient needs, develops and implements individualized care plans, and collaborates with a multidisciplinary team to ensure quality, culturally sensitive, and patient-centered care. Responsibilities include evaluating patient conditions, communicating updates, and adjusting care plans to promote optimal outcomes.
