About the Position

Job Title

Primary Care MD, NP, or PA – Per Diem (In-Home Health Assessments)

Location

Raleigh, NC (traveling/mobile within assigned counties)

Schedule / Job Type

Contract | Per Diem | Days | Monday–Sunday, 7:00 AM – 7:00 PM | 5–8 assessments per day

Key Responsibilities

• Conduct in-home health and wellness assessments (45–75 minutes each)
• Complete H&P evaluations including physical exam, BP checks, finger stick glucose, and urine sample collection
• Review and document medications and dosages
• Facilitate ancillary testing (A1C, spirometry, ECG, dilated eye exams, FIT) as applicable
• Document in iPad-based EMR; work independently with access to support hotline

Required Qualifications

• Active, unrestricted North Carolina license (MD/NP/PA)
• National certification required for NP/PA
• NPI required; CAQH profile required
• At least one weekday available per week (weekend-only availability not accepted)
• Willingness to travel (minimum 75 miles from home address) and cover a minimum of 3 counties
• Auto insurance meets requirements; must not have opted out of Medicare/Medicaid

Preferred Experience / Skills

• Geriatric and mobile/community-based care experience
