Licensed Practical Nurse
RAYVILLE NURSING & REHABILITATION
Looking to fill 3 positions, 2 for 7pm-7am and 1 for 7am-7pm. Job Summary Employee is responsible for the care of the residents in a designated area of the facility. Employee communicates with outgoing and arriving shifts of personnel to assure the continuity of quality of care. Makes rounds, evaluating the physical, emotional and social needs of the residents, ensuring these are met by the staff assigned to each resident group. Assesses staff ability to provide for each resident’s needs and intervenes as indicated. Supervises the care provided by the Nursing Assistants assigned to the unit, providing intervention and education as needed. Adjusts assignments as indicated. Prepares and administers medications and treatments according to procedures. Consults with physicians, receives records, transcribes orders as required. Makes rounds with physicians when necessary. Reviews resident care plans and ensures adherence. Documents records as per policy and procedures of the facility. Participates in the evaluation of employees working under his/her supervision and attends staff meetings and in‑services as required. The Registered Nurse, Licensed Practical Nurse or Licensed Vocational Nurse reports to the Director of Nursing and/or the Assistant Director of Nursing. Education Employee must be a graduate of an accredited school of nursing, currently registered with the state agency for nursing licensure, and hold a valid nursing license in the state of employment. Qualifications Demonstrates leadership and managerial ability. Has the ability to organize, plan and supervise care and the Nursing Assistants assigned to work at a designated unit and shift. Maintains effective interpersonal relationships. Is of good emotional, mental and physical health, having sound judgement and high professional standards. Must be willing and capable of providing emergency assistance to residents as indicated. Is courteous in working with residents, families and employees. Maintains a neat, well-groomed and professional appearance at all times. Has a desire to work with the geriatric residents and their current and prospective needs. Must be willing to be in the facility ready to begin assigned duties on time. Must be willing to be in the facility during weekdays as well as weekend hours. Must be willing to work preferred shifts as scheduled and assist on other shifts as needed to ensure adequate staffing for each resident. Must demonstrate ability to evaluate the geriatric resident’s needs and provide intervention timely to assure each resident’s needs are met. Must demonstrate ability to evaluate nursing staff needs and provide staff education timely to provide quality care. Must demonstrate ability to function as a cooperative team member with each member of the Nursing Department and other departments. Must demonstrate the ability to follow and supervise assigned staff to assure the facility’s policies and procedures are followed and upheld. Must demonstrate the ability to manage and supervise the assigned unit in a productive, timely manner. Must have a pre‑employment and an annual Tuberculosis screening examination. Responsibilities Receives report from the outgoing shift. Reviews communication systems to be knowledgeable of any unusual occurrences during the previous shifts. Assigns Nursing Assistants and other designated personnel, ensuring each resident’s needs will be met during the assigned shifts. Gives report to Nursing Assistants. Makes rounds of the assigned unit frequently during the shift, providing intervention as needed; remains attuned to the needs of the unit while performing other nursing tasks. Prepares and administers medications as ordered; reorders medications as needed for future shifts; assesses and documents medication issues per policy. Provides special procedures as ordered. Completes procedures as ordered, documenting results in detail; prepares a weekly skin report for the Director of Nursing; contacts the physician for any areas not responding positively and in a timely manner. Supervises the meal service for each resident on the unit, ensuring adequate nutrition. Continually assesses and reassesses residents for risk of complications such as skin breakdown, incontinence, contractures, reduced mobility, reduced participation in bathing, dressing or eating, weight loss, behaviors stemming from lifestyle change, etc. Observes the Nursing Assistants’ work to assure residents are turned, repositioned, toileted, walked, exercised, fed, hydrated, groomed, involved in their restorative program, and receives intervention as indicated. Observes the resident’s environment to maintain a clean, safe setting. Documents in nurse’s notes as per policy, ensuring accurate and timely records of any unusual observations, incidents or events. Reviews information, reassesses residents, interviews staff, evaluates documentation of each shift, and writes monthly nurse progress notes using the interdisciplinary care plan, noting progress or lack of progress toward resident goals. Suggests changes to the care plan in the “working copy.” Admits residents as required. Assists in ordering and reviewing laboratory test results as indicated; notifies the physician of results. Notifies the physician of unusual findings as indicated. Assists with auditing charts. Assists with answering and correcting consultant reports. Reviews, signs and provides intervention as indicated for Nursing Assistants’ documentation. Counts required medications for incoming and outgoing shifts. Makes appropriate notes in communication books to maintain adequate nursing communication. Assists in keeping other departments informed of resident change of condition as indicated. Assists in providing in‑services for assigned staff. Maintains the nurse’s station and medication room clean; assigns Nursing Assistants to keep ancillary rooms clean and orderly; assigns staff to keep equipment clean and in proper condition. Reviews and updates medication administration record, treatment administration record, and nursing care flow records for the upcoming month. Makes rounds toward the end of shift to ensure area is in order for the incoming shift. Gives report to incoming shift. Reviews and ensures adequate staffing for the incoming shift; replaces known absences promptly. Performs any other tasks as assigned by the Director of Nursing or Assistant Director of Nursing. Worker will be walking or standing almost consistently during each tour of duty as they supervise, assist, or attend to care. Bending/Stooping: Continuously. Worker will bend and stoop to assist residents, co‐workers, etc., almost consistently during each tour. Lift/Handling: Continuously. Worker will assist in fully lifting residents as needed; may lift up to 300 pounds with equipment or other personnel. Carrying: Continuously. Worker will carry equipment, weight range 1-50 pounds. Push/Pull: Frequently. Worker will push or pull medication carts, equipment, etc. weight range 1-50 pounds. Balancing: Frequently. Worker will balance when assisting residents to transfer or transport, giving medication or providing care. Twisting/Turning: Frequently. Worker will twist or turn when providing medication, assisting transfers, or helping staff with activities. Crouching/Stooping: Frequently. Worker will crouch and stoop when providing medication, assisting ADLs, or gathering equipment. Kneeling: Occasionally. Worker will kneel when providing medication, assisting ADLs, etc. Reaching: Frequently. Worker will reach when providing medication, ADLs, gathering equipment, or providing care. Handling/Manual: Continuously. Fine and gross motor skills required throughout the day. Speaking/Hearing: Continuously. Must communicate with residents, co‑workers, families, visitors, and physicians throughout the shift.
