Position Summary The Health Educator supports the Primary care team by providing health education to WellSpace Health patients. The ultimate goal of health education is to provide WellSpace Health patients with the necessary knowledge and skills to improve their well-being and live long healthy lives using a patient centered health approach. These services include health education to prevent and/or manage chronic and/or infectious diseases.

Required Qualifications
- High School diploma or equivalent, Bachelor's degree preferred.
- Certified Health Education Specialist (CHES) preferred.
- Health education/community health/public health degree or experience.
- Fluency in additional languages preferred (E.g Spanish, Farsi, Punjabi or Hmong).
- Able to conduct and facilitate one on one and group classes.
- Must be able to work collaboratively with others in a team environment, respecting and valuing the
- perspectives and experiences of others.
- Able to maintain and understand appropriate roles, and boundaries among clients, peers, and WellSpace partners/stakeholders.
- Able to function effectively in a fast paced environment.
- Ability to write progress notes and maintain detailed data records.
- Efficient time management skills, with the ability to meet deadlines and organizational needs.
- Demonstrated commitment to the provision of services for the underserved and sensitivity working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, HIV (AIDS) infected, and/or physiologically impaired.
- Must demonstrate a high level of verbal, writing and listening skills.
- Commitment to HIPAA and ability to abide by standards of professional ethics and maintain confidentiality.
- Current CPR and first aid certificate or willingness to be trained required within first (90) ninety days of employment.
- Computer literacy including use of Outlook email, Microsoft Word, Excel, and EHR.
- Adherence to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing.
- Assist and support the Center/Department/Program to meet standards of High Reliability.
- Must be willing to travel to the service locations and other health centers at least 10% of the time.
- Must be willing to successfully complete the credentialing process.
- Meet with each patient individually and/or in a group setting and provide health education.
- Implement health education programs within Primary Care Department.
- Implement a range of health education services, including establishing effective referral, scheduling and logistical support, and facilitating use of community services to promote the delivery of health education services.
- Collaborate with the patient and care team in the development of an individual health action plan and support the patient in the execution of their health goals.
- Refer patients to primary healthcare team, as appropriate.
- Compose and document progress notes daily into an electronic patient care system and maintain detailed data records daily.
- Provide support with the development of high quality, culturally appropriate health promotion materials.
- Track patient progress and program data daily for reporting.
- Attend regular monthly meetings with Health Centers and team.
- Perform other duties as assigned.
