Family Practice - Geriatrics Physician

• Seeking a BE/BC Family Medicine or Internal Medicine physician to join our Senior Health team
• Full-time and part-time positions available
• Fellowship training in Geriatric Medicine not required; Family Medicine or Internal Medicine candidates with a strong interest/experience in Geriatrics are welcome to apply
• Join an experienced team of Physician and Advance Practice Providers supported by clinic support staff: 1 RN and 1 Patient Service Rep
• EPIC EMR
• Nursing home admissions and rounding
• Opportunity available for Medical Directorship of three areas Nursing Homes for additional stipend related to the Medical Directorship responsibilities
• Average patients/day: 10
• Call schedule requirement: 5 days per month including weekends, call handled by phone
