Franciscan Health takes pride in hiring coworkers who provide compassionate, comprehensive care for our patients and the communities we serve.What You Can ExpectSign on bonus available (ask your Talent Acquisition Advisor if you are eligible for sign-on bonus)Minimum start pay $24.83 hourImplement nursing plan of care at the direction of the registered nurse.Provide home-based patient care based on physician orders and the established nursing plan of care.Collaborate with home health RN and multidisciplinary treatment team to individualize each patient's treatment needs, to achieve defined goals for improved patient outcomes.Conduct patient psychosocial assessments, to explore the physical, psychological, and social aspects of the patient and their conditions, obtain history, and elicit an accurate description of psychosocial needs.Contribute to developing discharge plans, identifying the post-discharge needs of the patient, and target appropriate resources for follow-up care, in collaboration with the patient, family and care team.Document patient care information, and ensure timely and accurate completion of patient care records in accordance with policies and procedures.QualificationsAssociate's degree or diploma from accredited licensed practical nurse program - required1 year acute care, long-term care or home health experience - required2 years home health experience - preferredLicensed practical nurse (LPN) - state licensing board - requiredCardiopulmonary resuscitation (CPR) - American Heart Association - requiredDriver's license - requiredAuto insurance - requiredTravel is required: up to 75%Equal opportunity employer.
