Registered Nurse Case Manager

Acute Care Case ManagerThis is a 26-week contract position.Day shift, 5x8 hours (08:00 - 16:30). Weekend rotation required.The Acute Care Case Manager will be responsible for coordinating patient care activities, including assessment, care planning, and discharge planning within an acute care hospital setting. This role requires effective collaboration with interdisciplinary teams to ensure patient needs are met throughout their care continuum.Conduct assessments and develop treatment plans based on individual patient needs.Coordinate care transitions and discharge planning effectively.Organize and participate in family meetings to discuss patient care plans.Assist with the arrangement of Durable Medical Equipment (DME), Skilled Nursing Facility (SNF) placements, and ambulance transport as needed.Ensure compliance with hospital policies and regulatory standards, including CMS guidelines.Minimum of 2 years of experience in Acute Care Case Management or Discharge Planning required.A California state license is required; pending licenses may be accepted as long as the candidate is licensed upon start date.Seasoned travelers only; first-time travelers are not accepted.Experience with patient ratios of 1:25 during the day shift.Strong knowledge of care coordination processes and regulations related to acute care.At ARMStaffing, we take care of our employees!
