This homecare position will focus on G-tube, Trach, and Vent care. Comprehensive paid training and ongoing clinical support are provided, making this a great opportunity to build your skills while delivering exceptional patient care.

Responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate patient care
• Initiate and follow the established plan of care, requesting revisions as patient needs change
• Provide therapeutic, preventive, and rehabilitative nursing interventions
• Monitor patient condition and communicate significant changes to the physician or supervisor
• Administer prescribed medications and treatments while documenting patient response
• Support patient and family education, care coordination, goal setting, and discharge planning as appropriate

Qualifications

• Current Licensed Practical Nurse (LPN) license in Illinois
• Current Health Certificate (per state regulations or contract requirements)
• Current PPD or Chest X-Ray
• Current BLS certification
• One year of LPN experience preferred
• One year of NICU/PICU experience preferred
