Occupational Therapist

Occupational Therapist (OT) Full-Time | Part-Time | PRN $10,000 Sign-On Bonus for Full-Time Position We are seeking a licensed, compassionate, and patient-focused Occupational Therapist (OT) to join our rehabilitation team and provide high-quality, skilled therapy services to residents in accordance with physician orders and individualized care plans. This is an excellent opportunity for an OT who is passionate about patient-centered care, clinical excellence, and helping residents regain function, increase independence, and improve their quality of life . Key Responsibilities Evaluate residents and develop individualized occupational therapy treatment plans based on clinical needs and physician orders. Provide skilled occupational therapy services in accordance with established care plans. Assess functional abilities and implement appropriate therapeutic interventions to promote independence. Monitor resident progress and adjust treatment approaches as clinically appropriate. Complete accurate and timely documentation, including evaluations, treatment notes, progress reports, and discharge summaries. Collaborate with nursing, therapy, and interdisciplinary team members to support positive resident outcomes. Participate in care planning, utilization review meetings, interdisciplinary conferences, and other clinical meetings as assigned. Educate residents, families, and staff regarding therapy goals, treatment plans, safety, and strategies that promote independence. Maintain established productivity, documentation, and quality standards. Support regulatory compliance, reimbursement requirements, and survey readiness. Promote resident safety, dignity, independence, and overall well-being. Qualifications Graduate of an accredited Occupational Therapy program. Current and active state license as an Occupational Therapist. Long-term care or skilled nursing experience preferred. Strong clinical documentation and communication skills. Excellent interpersonal, organizational, and time-management skills. Ability to work effectively within an interdisciplinary healthcare team. Compassionate, patient-focused, and committed to providing high-quality care. Physical Requirements Ability to stand, walk, bend, and assist residents throughout the workday. Ability to safely lift, transfer, and support residents during therapy activities. Ability to perform physical tasks associated with patient care and therapeutic treatment. Comfortable working in a clinical and long-term care environment. Ability to manage the physical and emotional demands of patient care.
