Licensed Vocational Nurse (LVN) PM Shift

Location 900 E Harrison Ave,Pomona, CA, 91767-2075,United States Base Pay $37.87 / Year Description Mt San Antonio Gardens is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to support our Independent Living residents. This role is responsible for responding to resident calls, handling campus-wide emergencies, and ensuring continuity of care for residents' medical needs. The LVN plays a key role in maintaining resident safety, coordinating care, and delivering high-quality nursing services in a senior living community. Schedule Evening Shift 3 p.m. - 10 p.m. Responsibilities Receives report from AM Clinic Nurse. Reads log book daily and follows through appropriately. Sets up and assists residents with routine medication. Home Health Aides/Med Techs may assist the residents with medications upon discretion of LVN. Makes house calls as needed and provides routine nursing treatments and administers first aid to the residents per Physicians orders. Reads all consultation sheets after a doctor's visit and follows through with any new orders. Administers injections and immunizations to residents and employees as needed. Completes Documentation: a. House Calls, Clinic Visits - should be checked on log sheet and electronic documentation as applies. b. Administers injections - Vitamin B-12, Insulin, Allergy, Hepatitis, and TB Tests, etc, as indicated. Document on appropriate forms. c. Incidents – Complete Incident reports, Investigation Summary and progress notes. Calls primary physician as needed and reports incidents and concerns you may have for residents. Obtains physicians orders and follows through appropriately. Coordinates the transferring of independent residents to and from the hospital, Magnolia Grove/Skilled Nursing, Villas, or the Transitional Care Room (TCR - Oak Tree Lodge (OTL). Assesses and coordinates the residents move to different levels of care as the condition presents itself and notifies all responsible parties when transfer occurs. Assesses residents’ condition and needs within one (1) hour of discharge from other levels of care or hospital to their independent living unit; will also complete proper documentation in a timely manner. Collaborates with AL and Skilled Nursing staff on IL resident's care and needs during temporary stays in these areas. Assesses employee injuries and follows through appropriately per MSAG guidelines. Responsible for the supervision of the Home Health Department: a. Monitor daily general routine care provided for the residents. d. Home Health Aide to be available to be in the Clinic area to assist LVN. Maintains supplies for the Clinic. Complete Relias training in a timely manner Reviews policy and procedure book frequently. Fax new prescriptions and renewal prescriptions to currently contracted pharmacy. Assures that all medication received from the currently contracted pharmacy is checked in, initialed and dated when received. Ensure delivery of medications from pharmacy to residents, as soon as possible after delivery to the Clinic. Assures that all medication is stored appropriately and locked up in the medicine cupboard or refrigerator in the Med Room. Is knowledgeable of standing Doctor‘s Orders (posted on door in the Clinic) Completes all charting and documentation appropriately and in a timely manner. Forwards copies of Durable Power of Attorney for Healthcare (DPAHC), Do Not Resuscitate (DNR), Physician’s Orders for Life Sustaining Treatments (POLST), Advocacy information (if on file) and emergency sheets when residents are admitted to the acute hospital, Assisted Living or Skilled Nursing. Reviews medication resident is taking and forward to staff at AL or Skilled Nursing. Checks on residents in the hospital in the absence of Admissions Coordinator and Clinic Director. Reports to the Out Calls nurse at the end of shift and any follow up of residents needed during the night. Completes other duties as assigned by supervisor. Practices good safety habits. Requirements California LVN license with two to three years’ experience Valid CA Driver’s license Knowledge of Geriatric Nursing care. Ability to maintain effective working relationships with doctors, residents and all employees. Ability to maintain professional relationships with residents and not discuss personal problems.
