Care Transport Associate NEMT Transport Driver

About the Role:
Sky Heart Home Care Services is looking for a compassionate and reliable Care Transport Associate to join our Non-Emergency Medical Transportation (NEMT) team in Apex, NC. This is a meaningful opportunity to make a real difference in the lives of patients by ensuring they arrive safely and on time to their medical appointments. - Safely transport clients to and from non-emergency medical appointments, therapy sessions, and healthcare facilities
- Assist passengers with boarding, exiting, and securing wheelchairs, walkers, and other mobility devices
- Maintain accurate trip logs, mileage records, and documentation in compliance with NEMT regulations
- Communicate professionally with clients, caregivers, dispatchers, and medical staff
- Conduct pre-trip vehicle inspections to ensure safety, cleanliness, and proper equipment function
- Follow scheduled routes while adapting to real-time changes or urgent transport needs
- Uphold patient dignity, confidentiality, and HIPAA compliance at all times
- Valid driver's license with a clean driving record
- Previous NEMT, medical transport, or patient care experience preferred
- CPR and First Aid certification (or willingness to obtain)
- Ability to assist clients with mobility challenges, including wheelchair securement
- Strong punctuality, time management, and organizational skills
- Compassionate, patient, and professional demeanor when working with elderly or disabled individuals
- Ability to pass a background check and drug screening
