Licensed Practical Nurse (LPN) – Continuing Care (Skilled Nursing / Rehab)

Location: Grand Rapids, MI 49508 (4118 Kalamazoo Ave SE)
Schedule: Days | 6:30 AM – 7:00 PM | 12-hour shifts | Every other weekend
Pay: $28 - $35 - DOE

Responsibilities

• Collect and monitor patient assessment data and report patient/family needs (physiological, psychological, sociocultural, spiritual) in coordination with the RN/provider
• Provide direct patient care under RN/provider direction, following facility policies, procedures, and unit routines
• Support implementation and evaluation of patient care plans to promote safety and continuity of care
• Communicate clearly with patients and families regarding treatments, medications, and equipment being used
• Document care provided in the electronic/paper medical record per guidelines and standards
• Collaborate with the interdisciplinary care team to ensure coordinated services and smooth handoffs
• Maintain a clean, safe patient environment; identify and report equipment concerns to appropriate staff
• Document patient chargeable items per standards
• Participate in quality monitoring activities and safety initiatives as assigned
  

Qualifications

• Active, current, unrestricted Michigan LPN license (required)
• BLS certification required (American Heart Association or American Red Cross; required within required timeframe per facility policy)
• High school diploma or equivalent (required)
• Completion of an approved nursing program (required)
• Successful completion of a competency-based orientation program (required)
• Healthcare experience (preferred)
• Comfortable with computer-based documentation and detailed, accurate charting
• Strong communication skills and professional bedside manner in a continuing care environment
