APRN- Family Medicine
The Nurse Practitioner coordinates the planning, implementation and on-going monitoring and evaluation of patients of all age ranges, under the supervision of a collaborating licensed physician. Responsibilities include examining, diagnosing, treating patient conditions, educating patients and family members about medical options, prescribing medications and treatments, and performing other duties to support patients.
General Duties, Tasks and Responsibilities
- Work independently with other members of the medical team that take part in an interdisciplinary team that provides primary care to employees and patients
- Prescribe medications following the clinic's policy, formulary and relevant regulations of the State
- Performs and documents mental health assessments, consults as needed with the provider/practitioner concerning the plan of care.
- Maintains current knowledge of community resources, how to access them and the regulations for access to them.
- Communicates effectively in both the patient chart and orally with the staff so as to ensure that can be understood and executed by others as appropriate.
- Complies with all established safety procedures to ensure a safe environment for patients, visitors and staff.
- Participates in performance improvement activities.
- Performs other duties as assigned.
Education Requirements
- Master's degree in Nursing from a state-accredited school required
Certification/Licensure Requirements
- Current IL APRN Licensure
- Current IL DEA License Required
- BLS Required
Experience Requirements
- 2 years Family Practice experience preferred
Computer Skills
- Strong computer skills with the ability to navigate EMR programs

- Be compassionate, sensitive, and possess strong reasoning and thinking skills
- Ability to work independently, prioritize and complete tasks in a timely manner
- Excellent ability to thrive under pressure and adapt to changes in assignment
- Strong communication skills including reading, writing and the ability to listen effectively
- Ability to work within the confines of confidentiality and maintain professional boundaries
