PRN Registered Nurse (RN) – Adult Home Health

Colorado Springs, CO
$47–$52 per visit | PRN | Flexible Scheduling

We are looking for an experienced PRN Registered Nurse (RN) to provide skilled home health visits to adult patients throughout Colorado Springs .

This is a flexible PRN opportunity where you can pick and choose the visits that work for your schedule and preferred area of town . There is no requirement to take every available patient or build a full-time caseload.

OASIS charting experience is required.

Responsibilities

The RN provides skilled nursing services in accordance with each patient's individualized plan of care, including:

• Complete skilled nursing visits in patients' homes
• Perform comprehensive nursing assessments
• Complete OASIS documentation and charting
• Perform Start of Care, Recertification, Resumption of Care, Transfer and Discharge documentation as assigned
• Monitor patient condition and identify changes requiring physician or clinical team notification
• Provide medication education, disease-management education and caregiver training
• Perform physician-ordered skilled nursing treatments and procedures
• Develop, implement and update nursing plans of care
• Coordinate care with physicians, therapists, CNAs and other members of the healthcare team
• Obtain and process physician orders as needed
• Complete accurate and timely clinical documentation
• Participate in discharge planning and care coordination
• Provide supervision of CNAs/LPNs when required by the patient's plan of care and agency policy
• Follow all applicable state, federal and agency home health requirements

Available visits are offered throughout Colorado Springs. Nurses can select visits based on their availability, preferred geographic area and desired workload .

Pay: $47–$52 per visit

Requirements:

 

Qualifications

• Current Colorado Registered Nurse (RN) license
• OASIS experience required
• Ability to independently complete home health documentation and assessments
• Strong assessment, communication and time-management skills
• Comfortable traveling to patient homes throughout the Colorado Springs area
• Adult home health experience strongly preferred
