LPN/LVN – Long-Term Care

Location: Utica, NY 13502
Duration: 13 weeks with a high possibility of extension
Shift: 10:30 PM–6:30 AM
Schedule: 37.5 hours/week
Break: 30-minute unpaid break

Position Overview

We are seeking an experienced Licensed Practical Nurse (LPN/LVN) to provide nursing care in a long-term care environment. The ideal candidate will have previous LTC experience, travel nursing experience, and proficiency with PointClickCare EMR.

Key Responsibilities

Provide nursing care to residents in a Long-Term Care (LTC) setting.
Administer medications and treatments according to established orders and protocols.
Monitor residents' conditions and report changes to the appropriate healthcare team.
Maintain accurate and timely nursing documentation.
Communicate effectively with residents, families, physicians, and other members of the healthcare team.
Follow facility policies, procedures, safety standards, and infection-control practices.
Utilize PointClickCare EMR for patient documentation and clinical records.
Promote resident safety, comfort, dignity, and quality of care.
Collaborate with nursing staff and other healthcare professionals to support resident needs.

Required Qualifications

Minimum 1 year of experience in a Long-Term Care (LTC) facility .
Previous travel nursing experience with supporting documentation required.
Active New York State LPN license required.
BLS-AHA certification required.
High School Diploma required.
Experience with PointClickCare EMR required.
Strong communication, clinical assessment, and organizational skills.
Ability to work independently and effectively as part of a multidisciplinary healthcare team.

Schedule & Compensation

Night Shift: 10:30 PM–6:30 AM
37.5 hours/week
30-minute unpaid break
Contract: 13 weeks with a strong possibility of extension
