About the Role

As a Physical Therapist at Care Pavilion Nursing and Rehab, your responsibilities will include:

• Conducting thorough evaluations and developing individualized treatment plans to address each patient's unique needs and goals

• Implementing and modifying treatment plans as needed to ensure optimal patient outcomes

• Collaborating with other members of the interdisciplinary care team to ensure comprehensive care

• Maintaining accurate and detailed records of patient progress and treatment plans

Responsibilities

• Conduct thorough evaluations and assessments of patients to identify treatment needs

• Develop and implement individualized treatment plans to address patient needs and goals

• Provide physical therapy interventions, including exercises, stretches, and other modalities as needed

• Monitor and record patient progress, adjusting treatment plans as necessary

• Participate in care conferences and interdisciplinary team meetings to ensure comprehensive care

Education and Experience
