See our Benefits Page HERE. 401K Employer Match up to 4% Competitive Time Off Plans About Our Program Pacific Clinics will be opening a high‑intensity, community‑based Adult Full Service Partnership (FSP) program for adults age 18+ with serious mental illness (SMI) and complex needs. The program delivers “whatever‑it‑takes,” recovery‑oriented services primarily in the community to stabilize individuals, reduce hospitalizations and incarceration, prevent homelessness, and support long‑term recovery and community integration. The ACT FSP (Assertive Community Treatment - Full Service Partnership) program is an intensive, community‑based mental health service for 96 individuals with serious mental illness who have high needs. It provides wraparound, multidisciplinary support, including psychiatry, therapy, case management, crisis intervention, housing assistance, and rehabilitation, delivered with a community driven approach. The goal is to reduce hospitalization and incarceration, promote recovery, support housing stability, and improve quality of life through continuous, personalized care. This program will also serve those who are involved in the justice system as well. Schedule Monday - Friday 9 am - 6pm and as needed evenings, weekends and holidays, or to support a crisis Pay Range $70,304 - $84,419 Job Summary Processes internal and external referrals under the supervision of the Clinical Program Manager. Conducts and coordinates intakes, engages with customers, manages response to agency referrals, and enters and maintains electronic health record (EHR) information as well as any applicable County systems. Performs interviews with potential clients and completes all intake paperwork. Completes all required administrative paperwork and data entry. Supports program operations, management, agency compliance and quality assurance. Acts as liaison between family, Social Worker, referring agency, and Clinical Program Manager. Responsibilities Receive and process all incoming referrals from various referral sources. Complete all intake paperwork; accumulating and distributing all intake information to appropriate parties. Contact customers and/or Clinical Program Manager or designee to screen for eligibility and/or appropriateness of referral for service. Complete an Initial Mental Health Assessment as needed. Initiate treatment plan document for sign-off by LPHA. Provide training and guidance on the intake and assessment process. Scan and submit billing information for private insurance customers. Develop and maintain tracking systems. Skilled user of Agency EHR and access to County system (e.g. FENICS). Prepare and maintain enrollment packet paperwork and department forms. Liaison between family, placement worker, Case Manager and Clinical Program Manager, and County contract monitor. Utilizes whole person care perspective (e.g. empathetic, compassionate active listening) to gather required information in an external and productive manner. Respond to and complete service requests (e.g. calls, emails, etc.) and enter required information in EHR per business protocol. Provides information to customers, caregivers, guardians and referral sources regarding the admissions process and available services. Attend weekly departmental meetings. Meet with Supervisor to update training needs of team and to perform assigned duties as required. Other related responsibilities, as assigned, to support specific department/business needs. Qualifications Bachelor's degree (B.A.) in the field of Psychology, Social Work, or related field required and one (1) year of related experience, or equivalent combination of education and experience (two years of relevant experience equals one year of education) to include: At least one (1) year experience working in a similar medical or behavioral health environment.
