Avera Platte Health Center is seeking a qualified Patient Care Coordinator to join Platte Care Center. You will support daily nursing management, supervise CNAs and RNs, and lead care and safety committees to ensure high-quality resident care. You will coordinate admissions, conduct daily rounds, and manage MDS 3.0 scheduling and care plans, while upholding infection prevention standards and patient rights.
