Nurse Practitioner in Syracuse, NY

Overview TeamHealth is looking for a nurse practitioner (NP) or physician assistant (PA) to work full-time (5 days per week) doing primary care rounding in a skilled nursing facility in the Syracuse, New York area. You will have the ability to set up your own daily schedule and autonomy over your practice, with uncapped earning potential and the opportunity to build a position that matches your lifestyle and career goals. Key Responsibilities Perform primary care in a post-acute setting for short term and long-term patients Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care Order and interpret lab results Prescribe and monitor medications Consult with physicians and coordinate patient care with other healthcare practitioners Educate patients and their caregivers on coping skills to handle chronic and age-related conditions Build relationships with patients, patients\/family members, and facility staff to facilitate effective treatment delivery Promote healthy lifestyle changes Requirements A minimum of 1 year experience as a nurse practitioner in a primary care setting required Post-acute care experience a plus, but not required Active and unrestricted New York NP or PA license required Active and unrestricted DEA with prescriptive authority required Compensation Estimated compensation is salary range of $115,360 to $122,416 with no cap on productivity income potential.
