Assignment Details
- Duration: 16 weeks
- Schedule: Monday–Friday
- Shift: 8:00 AM – 5:00 PM
Requirements
- Active California RN license
- CPR or BLS certification (AHA, HS‑BLS, or RQI‑BLS per policy)
- Epic experience required
- Minimum 2 years of Home Health or Care Management experience
Responsibilities
- Perform comprehensive patient assessments in the home setting
- Develop, implement, and update individualized plans of care
- Coordinate care with physicians, therapists, and interdisciplinary teams
- Provide skilled nursing visits and patient education
- Monitor patient progress and document outcomes accurately
- Ensure compliance with home health regulations and standards
- Assist with coverage at the Glendale location as needed
