Physical Therapist

As a Physical Therapist, you will provide direct patient care, evaluate functional needs and outcomes, and consult with other specialists. You will design and implement individualized care plans and incorporate patient participation whenever appropriate.

• Promotes Elara Carings philosophy, mission statement and policies to ensure quality of care.
• Provides direct patient care, evaluates functional needs, and updates plans to ensure optimal outcomes.
• Designs and implements care plans based on thorough assessments.
• Directs support staff to achieve patients functional goals.
• Documents evaluations, goals, and maintains required documentation.
• Provides direction and leadership within the team and promotes physical therapy in the community.
• Full range of body motion including handling, lifting, and transfer of patients.
• May require irregular hours or on-call work.

Requirements:

• Graduate of an APTA-accredited physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Willing and able to travel within branch/office coverage area (up to 50%).
• Must have valid drivers license, dependable vehicle, and current auto insurance.
• Ability to frequently lift, push, pull, and support up to 50 pounds.
