North Smithfield School Department Job Description POSITION TITLE: SCHOOL NURSE TEACHER Reports to: Building Principal Approved by: Superintendent of Schools Date: December 7, 2009 POSITION SUMMARY:

• Provide professional nursing services in accordance with the Nurse Practice Act, Rules and Regulations for School Health Programs of Rhode Island, Department of Health/Department of Education. School nursing services enable students to access academic programs, in the least restrictive environment, through safe and appropriate management of health conditions which support student academic success.

EVALUATION:

• In accordance with provisions of the School Committee Policy and NSTA Contract.

TERMS OF EMPLOYMENT:

• School year. Salary and other conditions per contractual arrangement with the School Committee.

MINIMUM QUALIFICATIONS:

1. Valid Rhode Island license to practice as a registered nurse.
2. Rhode Island School Nurse-Teacher Certification
3. Three years nursing experience

Essential Duties, Functions, and Responsibilities

HEALTH SERVICES Provide professional nursing services in accordance with Nursing Standards and Rhode Island Licensing. Identify students' health needs, implement services, and evaluate plan of care. Provide for state-mandated screening programs including vision, hearing, dental, and scoliosis. Maintain individual student health records. Member of the school's multi-disciplinary team, interpreting medical data and writing objectives for the health component of the IEP and/or 504. Support mandated immunization protocol from the RI Department of Health. Prepare and submit reports as needed for the Superintendent and RI Department of Health. Detect, plan, treat, and provide referrals and follow-up on health problems interfering with children's learning. Provide for routine and as needed treatments in accordance with the student's Individualized Health Care Plans. Provide for safe administration of medications. Visit student homes when necessary. Documentation of health services provided in accordance with RI State mandates. Perform other duties as assigned by the Principal and/or Superintendent of Schools.

HEALTH EDUCATION Collaborate with other educators and community agencies as a resource in health education for school personnel, students, and families.

HEALTHY AND SAFE ENVIRONMENT PROFESSIONAL DEVELOPMENT Assumes responsibility for continuing education and professional development to assure updated skills. Participates in professional organizations that enhance School Nurse-Teacher practice. Contributes to the professional growth of others by providing appropriate in-service as needed.

Job Requirements

• This position's assignment will be North Smithfield Elementary School (Gr. Pre-K - 4). Must hold a RIDE School Nurse Teacher certificate. Please apply through School Spring. Hard copies of applications are not accepted unless requested.
