Registered Nurse Float - Urgent Care

HealthPartners is hiring for a Registered Nurse Float in Urgent Care. This position will float to Woodbury, Arden Hills, Apple Valley, St. Paul Como and St. Paul Wabasha. More clinics to be added in the future.

QUALIFICATIONS

REQUIRED

Graduate of an accredited school of nursing Current RN licensure in Minnesota Current RN licensure in Wisconsin within two months of hire Current BLS certification for health professionals through the American Heart Association or Red Cross required and maintained. On-line BLS certification is not acceptable Strongly demonstrated written and oral communication skills

Performance is up to standard when: Uses health coaching techniques to elicit patient goals and priorities to develop individualized patient action plans Uses population management tools such as registries to identify high risk patients and track, monitor and follow-up with their care Engages, assesses readiness to learn and educates patients and families to implement strategies to reach a desired goal or care intervention Identifies and secures patient services needed for patient care Informs and supports patients and families to assist them in making informed decisions and navigate the healthcare system. Education Definition: Education imparts knowledge designed to develop skills, to enhance decision-making, and to promote understanding. It includes interaction with the patient, family and community, as well as participating in formal and informal professional education.
