Housing Case Manager - Permanent Supportive Housing (PSH)

A community action organization in Connecticut is seeking a Case Manager to establish relationships with clients and develop comprehensive service plans. The ideal candidate will have a Bachelor’s Degree in social work or equivalent experience, along with strong computer skills. Responsibilities include assisting clients with benefits and conducting home visits. An important aspect of the role is ensuring client advocacy and crisis intervention while maintaining accurate records.
