Position Summary We are seeking a Physician (MD) to provide mobile primary care and rounding services across senior living communities. You will manage a panel across assisted living, SNF, memory care, and long‑term care settings, with a focus on chronic disease management, transitional care, and preventative medicine. This role is designed for physicians who want to work within a home‑based primary care model while influencing outcomes across a defined territory. Responsibilities Provide on‑site primary care and geriatric rounding in assisted living, memory care, and SNF settings. Manage chronic conditions, transitional care, and preventative health initiatives. Reduce hospitalizations and ER utilization through proactive care delivery. Perform diagnostics, treatment planning, and medication management. Collaborate with NPs, PAs, nursing staff, therapists, and case managers. Serve as a clinical leader within post‑acute and long‑term care environments. Ensure accurate EMR documentation, care coordination, and follow‑up. Qualifications Active Minnesota Physician (MD) license in good standing. Experience in geriatrics, internal medicine, family medicine, or post‑acute care preferred. Familiarity with value‑based care, SNF rounding, or home‑based primary care models. Strong ability to manage complex patient populations and work independently. Valid driver’s license and reliable transportation.
