We are seeking a Home Health Registered Nurse to provide medical care at home, including medication management, wound care, health monitoring, and coordination with physicians to ensure safe recovery. This is a great opportunity to join our established home health agency, which has served the community for over 10 years.

Responsibilities

• Assess patient health status and record medical histories.
• Develop, implement, and evaluate individualized nursing care plans.
• Administer medications, injections, and treatments as prescribed.
• Monitor patient vital signs and respond to changes appropriately.
• Educate patients and families on health conditions and post-care requirements.
• Collaborate with physicians and healthcare professionals to coordinate care.
• Maintain accurate and confidential patient records and documentation. 

Required Qualifications

• Current and valid Registered Nurse (RN) license (State of Michigan) is required
• Experience in home health or community nursing is preferred
• Strong communication and critical thinking skills.
• Ability to handle physical demands of nursing care.
• Previous experience with electronic medical record (EMR) systems.
