Pharmacy Technician

Outpatient Services:

Provides professional pharmacy services to the community. Reviews, interprets, and dispenses medications, drugs, and other pharmaceuticals as prescribed by physicians/providers according to professional standards, state and federal laws and regulations. Counsels’ patients on medication use, provides drug information to patients, licensed personnel. Other duties as necessary and as assigned.

Skilled Nursing Facility:

Performs monthly medication regimen reviews (MRR), prepares quarterly summary of trends and findings, attends quarterly CQI (Continuous Quality Improvement) meeting. Provides drug information to medical and nursing staff. Provides drug safety education in-services to SNF department. Provides support to SNF DON (Director of Nursing) where necessary. Provides services necessary for meeting state and federal laws and regulations. Other duties as assigned.

Inpatient Services:

Reviews and interprets medication orders. Reviews medication profiles to prevent adverse drug reactions, therapeutic duplications, dosing errors, and drug-drug interactions. Monitors therapeutic treatments and relevant lab values and parameters. Adheres to protocols and performs therapeutic interchanges. Provides drug information to medical and nursing staff. Provides support to overall patient care and safety. Supervises ancillary staff such as pharmacy technicians and/or clerks. Performs projects as assigned necessary for a functional and efficient pharmacy.

Job Duties and Responsibilities:

· Perform clinical interventions and monitors therapies.

· Provides drug information to healthcare team members and counseling patients on drug therapies when necessary.

· Participate in clinical programs.

· Accesses patient specific data when performing medication verification. Demonstrates knowledge and understanding of therapeutic drug monitoring.

· Performs patient care in a manner that promotes the best outcomes for patients using patient-specific data/information to minimize potential medication errors.

· Demonstrates knowledge and proficiency in renal function calculation and dosing adjustment.

· Verifies and prepares medications by reviewing and interpreting physician and provider orders, detecting therapeutic appropriateness.

· Dispenses medication by compounding, packing, and labeling appropriately.

· Organizes and directs ancillary staff workflow, verifying their work.

· Verifies order entries and verifies daily charges.

· Performs outdate inspection of all medication-containing areas.

· Comply with state and federal laws and regulations set by California Board of Pharmacy, Drug Enforcement Administration (DEA), and Food and Drug Administration (FDA).

· Maintains a safe and clean working environment by complying with policies and procedures, rules, and regulations.

· Maintains pharmaceutical knowledge by attending Continuing Education (CE).

· Provides the best and professional service to customers (including colleagues and coworkers).

· Gains proficiency with use of CPOE (Evident System) and Medication Dispensing Unit (PYXIS).

· Attend code blue when on duty.

· Gains proficiency with use of medication error reporting system.

· Communicates clearly and appropriately with medical staff, nursing staff, and the public.

· Documents clinical interventions and monitoring when applicable and necessary.

· Maintains department goals in meeting state and federal laws, regulations, and requirements.

· Demonstrates knowledge and understanding of current Pharmacy and Therapeutics Committee (P&T) approved pharmacy protocols.

· Knows current expected daily workflow and daily duties.

· Performs other duties as assigned.

Professional Requirements:

· Adheres to dress code, appearance is neat and clean, wears identification while on duty, and reports to work on time as scheduled. Use of the computerized time system correctly.

· Completes annual education requirements, attends annual review, continuous quality improvement activities (CQI), staff meetings and performs departmental in-service.

· Always maintains and ensures patient/resident and employee confidentiality.

· Perform duties in a safety-conscience manner to eliminate or reduce the likelihood of work-related injury or illness. Maintains awareness of staff adherence to safety procedures.

· Prioritizes and flexibility in organizing and adapting to changing needs of the department to provide the best patient care and efficiency.

· Represents the organization in a positive and professional manner.

· Works at maintaining a good rapport and a cooperative working relationship with physicians, departments and staff.

· Complies with all organizational policies regarding ethical business practices.

· Communicates the mission, ethics, and goals of the facility, as well as the focus statement of the department.

Regulatory Requirements:

· Doctorate of Pharmacy degree required.

· Current California Pharmacy License.

· Minimum of one-year of pharmacy experience.

Language Skills:

· Ability to effectively communicate in English; both verbally and in writing.

· Strong written and verbal skills.

· Additional languages preferred.

Skills:

· Possesses basic computer knowledge.

· Proficient in using Automated Dispensing Unit, IV Laminar Flow Hood, eHR systems.

· Self-directed, motivate, able to prioritize work, and capable of supervising support personnel.
