Occupational Therapist (OT) Home Health - Palm Beach County, FL

Job Location: Boca Raton, FL 33487 PRN Occupational Therapist (OT) – Home Health Position Summary We are seeking a compassionate and self‑motivated Occupational Therapist (OT) to join our Home Health team on a per diem (PRN) basis. In this role, you will provide individualized, in‑home therapy services to help patients regain independence in their daily living and self‑care activities. Responsibilities Perform initial and ongoing assessments of patients’ functional abilities, ADLs, mobility, safety, and home environment. Develop and implement individualized care plans based on physician orders and patient needs. Deliver therapeutic interventions including training in self‑care, use of adaptive equipment, and home safety modifications. Educate patients and caregivers to support recovery and independence. Accurately document all care provided using WellSky. Coordinate care and communicate effectively with the interdisciplinary home health team. Maintain compliance with agency, state, and federal guidelines. Qualifications Current FL licensure as an Occupational Therapist (OT). Minimum 1 year of clinical experience; home health experience preferred. Familiarity with WellSky EMR or similar documentation systems preferred. Strong clinical reasoning, time management, and communication skills. Ability to work independently and manage a flexible field‑based schedule.
