Social Worker We are seeking a dedicated and compassionate Social Worker to join our team, a 235-bed facility located in Berkeley Township, New Jersey. As a Social Worker at our facility, you will play a vital role in providing long-term skilled nursing care and short-term post-acute rehabilitation to our patients. About the Role: As a Social Worker, you will work closely with our interdisciplinary team to provide high-quality care and support to our patients. Your responsibilities will include: Conducting assessments and developing care plans to meet the psychosocial needs of our patients Providing counseling and support to patients and their families to address emotional, social, and psychological issues Facilitating communication and coordination between patients, families, and healthcare providers Identifying and addressing social determinants of health that impact patient outcomes Collaborating with other healthcare professionals to develop and implement individualized treatment plans Responsibilities: Provide psychosocial assessments and develop care plans to meet the needs of our patients Develop and implement individualized treatment plans in collaboration with other healthcare professionals Provide counseling and support to patients and their families to address emotional, social, and psychological issues Facilitate communication and coordination between patients, families, and healthcare providers Identify and address social determinants of health that impact patient outcomes Requirements: Bachelor's or Master's degree in Social Work (MSW) from an accredited program Minimum of 2 years of experience in a healthcare setting, preferably in a social work role Strong communication, assessment, and intervention skills Ability to work effectively in a fast-paced environment and prioritize tasks efficiently Strong knowledge of social work principles, theories, and practices What We Offer: The opportunity to work with a dedicated and compassionate team of healthcare professionals A dynamic and supportive work environment that fosters growth and development #J-18808-Ljbffr
