Title: Case Manager/ Advocate -Full Time Onsite 

Department: Counseling

  Work Week: 40 hours Three days 8:30 am- 4:30 pm; Two days 12-8 pm

Status: Nonexempt

Reports to: Chief Operating Officer (COO) 

Job Summary : Will provide trauma informed case management, housing services, intervention, crisis management, intake, assessment, goal planning and referrals to victims of abuse and their dependent children to residents and/or their children living at DVCCC’s emergency shelter and transitional housing program. Work with other agencies to support survivors accessing and maintaining housing. 

Job Responsibilities: 

• Maintain strict confidentiality of all client identification and information. May disclose confidential information only with valid, informed consent from a client or party legally authorized to consent on behalf of client.
• Provide intensive case management services to clients in need – focusing on self-sufficiency, housing needs, advocacy, trauma-informed care, and hands-on assistance.
• Assists DVCCC residents and victims of domestic violence in locating and securing housing. Provides follow up care and supports to assist clients maintain housing.
• Develop case management plans including safety planning with survivors, identifying existing barriers, complete needs assessment, family/individual service plans.Meet with clients on a regularly scheduled basis to discuss, assess and assist in the removal of barriers and meeting of case management goals.
• Maintain knowledge of current resources to coordinate care and make referrals/advocate to appropriate providers and resources. Provides direct referrals for victims to internal and external resources including housing, legal, counseling, and mental health/substance use services.
• Attend case conferences with Chester County Open Hearth, Housing Authority of Chester County and works in collaboration to develop and maintain resources that assist residents with goals toward self-sufficiency. 
• Coordinates emergency financial assistance which includes payment for taxis, food, transitional and permanent housing, clothing, and emergency safety measures. 
• Educates clients about Victims Compensation Funds, Relocation Assistance, Moving Ahead, and other financial resources available. 
• Completes and/or reviews housing intakes to assess and evaluate residents for admission. Ensures all shelter spaces are prepared to receive guests. Provides orientation (including client handbook, procedures, and tour) at the time of intake.
• Prepares discharge summary, paperwork, and departure information at the time of transition for each client.
• Maintain client files and client documentation in ETO including HMIS, Shelter/Bridge intake and exit data. 
• Regularly assess the safety and well-being of clients. 
• Facilitates/co-facilitates psychoeducational, support groups, and housing groups. 
• Assists with daily operations of the housing program.
• Participate in hotline line back up and on-call rotation as needed.
• Perform other tasks as assigned and required by program goals and objectives.
• Assists with administering satisfaction surveys. 

REQUIRED TRAINING

• Attend a minimum of 10 hours of training annually. 
• Assist with training other individuals in the Social Service Systems.

  TEAMWORK

• Document program participant information in the database system daily.
• Attends staff meetings, other required meetings, and in-service trainings.
• Participates in all agency-wide events: 45-hour advocate training; October awareness; volunteer recognition; various celebrations; fairs, etc.
• Assist with accomplishing agency-wide annual goals and objectives.
• Assist with keeping the facility clean and neat.
• Work collaboratively with social service agencies, hospitals, clinics, and other community resources to assist with external coordination of services for victim/survivors.

  SKILLS AND QUALIFICATIONS

• Able to work under pressure and adhere to deadlines.
• Strong commitment to the mission and goals of DVCCC, professional work goals and personal development.
• Through knowledge and understanding of issues confronted by victims of abuse.
• Able to work with victims of domestic abuse and their children. Able to work with a diverse population. 
• Knowledge of housing systems, and community agency support services available to victims of domestic violence.
• Ability to speak to program participants and various audiences.
• Ability to strategize issues related to domestic violence.
• Ability to establish effective working relationships with staff, volunteers, and a variety of internal and external colleagues.

Physical Requirements: 

• Able to move and lift supplies, furniture, etc. up to 40(forty) pounds. This position requires visual and auditory acuity and the physical ability to climb stairs, stand for long periods of time, and walk distances.

Other Requirements

• Computer proficiency in Microsoft Office Products.
• Must have transportation to travel to meetings and trainings.
• Must be willing to work flexible hours to meet program needs and grantor accountability requirements.
• Adhere to all agency policies and procedures (attendance, work hours, paperwork, etc.)
• Childcare, criminal and FBI clearances as required by the state and DVCCC.

Environment and Conditions

The incumbent’s typical place of work is an office environment and thus will not be substantially exposed to adverse environmental conditions.

  Minimum Training and Experience

• Bachelor’s degree in social services or related field 
• Three years case management experience
