Shifts: 12 hours, 6p-6a; every other weekend and holiday

Key Responsibilities of the LPN/LVN - Long Term Care - Travel

• Administer medications and treatments in accordance with prescribed orders
• Monitor patient's condition and notify care providers of any significant changes
• Maintain accurate patient records using Point Click Care (PCC) system
• Ensure patient comfort and uphold health and safety regulations
• Collaborate with the healthcare team to provide a high level of patient care

Requirements of the LPN/LVN - Long Term Care - Travel

• BLS Certification
• Valid LPN/LVN license
• Experience in long term care settings preferred
