POSITION SUMMARY - RN Case Manager


POSITION DUTIES - Provides patient-centered, holistic care by assessing patient and family needs, developing and implementing individualized care plans, evaluating outcomes, and adjusting interventions as needed. They serve as an advocate and educator for patients and families, coordinate care across interdisciplinary teams, facilitate communication and referrals, delegate appropriate tasks, and promote safe, efficient, and high-quality outcomes throughout the continuum of care.


MINIMUM REQUIRED QUALIFICATIONS -

• CA RN license
• CPR/BLS
• 3+ years recent RN experience working in RN role
• 2+ years recent RN experience working in case management

LENGTH OF ASSIGNMENT - 13 weeks


SHIFT / HOURS PER WEEK - 8-430, every other weekend required


START DATE - 9/21/2026
