Inpatient Case Manager-MSW

Inpatient Case Manager MSW – Complex Cases Sentara Obici Hospital, Suffolk, VA, is seeking an experienced Inpatient Case Manager MSW to focus on complex cases. Position Type: Full Time – Day Shift (40 hours/week) – Monday to Friday 8:30 a.m. to 5 p.m. with weekend rotation approximately every 4 weeks and holiday/on‑call commitments. Responsibilities Responsible for the provision of medical social work services to patients receiving care in the hospital setting. Assesses the social, cultural, environmental, and financial situations, as well as the disposition needs of each referred patient. Collaborates with healthcare team members and community organizations to identify needs and resources in the hospital and community. In consultation with the healthcare team develops effective transition plans. Collaborates with the RN Case Manager to ensure a smooth flow of patients through their hospital stay. Education Master's Level Degree in Social Work Certification / Licensure Certification in Case Management (CCM, CMAC or ACM‑SW) required within one year of eligibility BLS required within 90 days of hire Experience 1 year of related acute care and/or healthcare experience required. Educational internship experience in an acute care facility can be considered. Healthcare experience refers to the care delivered in a hospital or an acute care facility.
