Personal Care Worker (PCW) – Part-Time PM Shift

Part-Time | SilverLeaf Assisted Living | Augusta, WI

Shift: PM Shift | 2:00 PM – 10:00 PM
Starting Wage: $17.00/hour

SilverLeaf Assisted Living is seeking a compassionate and dependable Personal Care Worker (PCW) to join our PM shift team. As a PCW, you'll have the opportunity to build meaningful relationships with residents while providing the support, encouragement, and care they need to enjoy a safe and fulfilling lifestyle.

What You'll Do

• Provide compassionate, person-centered care to assisted living residents
• Assist residents with activities of daily living (ADLs), including personal care, dressing, grooming, toileting, and mobility
• Support residents with meals, evening routines, and daily activities
• Encourage resident independence while providing assistance when needed
• Observe residents and report changes or concerns to appropriate team members
• Maintain a safe, clean, and welcoming environment for residents
• Build positive relationships with residents, families, and team members

• Compassionate, patient, and dependable individual
• Strong communication and teamwork skills
• Ability to provide respectful and dignified care
• Willingness to support residents with their daily needs
• Must be 18 years of age or older
