$55 - $75 hourly

Responsibilities:

• Develop and implement personalized care plans tailored to each patient's unique needs and goals.
• Collaborate with an interdisciplinary team to ensure comprehensive and coordinated patient care.
• Conduct thorough assessments to evaluate patients' physical, emotional, and social needs.
• Provide hands-on therapy to enhance patients' daily living skills and promote independence.
• Educate patients and their families on therapeutic techniques and strategies for continued progress.
• Maintain accurate and timely documentation of patient progress and therapy sessions.
• Adapt therapy interventions based on patient feedback and evolving needs to ensure optimal outcomes.

Qualifications:

• Experience in conducting comprehensive patient evaluations, focusing on activities of daily living, mobility, and cognitive function.
• Ability to develop and implement personalized treatment plans that align with patient goals and physician directives.
• Proven track record of delivering skilled occupational therapy interventions to enhance patient independence and safety.
• Familiarity with conducting home safety assessments and recommending appropriate adaptive equipment.
• Strong communication skills to effectively educate patients and caregivers on disease management and assistive device usage.
• Proficiency in completing required documentation, including OASIS assessments and progress notes, accurately and promptly.
• Experience in collaborating with interdisciplinary teams to ensure cohesive patient care and successful outcomes.
