Requirements

Must Have:

• 2+ years of acute hospital Case Management experience.

• Open to first-time travelers!

• Background in MS, SDU/PCU/IMC/Obs, or Surgical Services.

Skills Required:

• Care Coordination & Discharge Planning.

• Disease Management & Needs Assessment.

• Ordering DME & planning Plan of Care.

• Patient/family education, resource referral, and team collaboration.
