Duties include but not limited to:

• Works collaboratively in an interdisciplinary coordinated care delivery environment (i.e., patient aligned care team, or other specialty care.
• Review provider templates, clinic grids, testing requirements, and capacity to ensure timely access to care.
• Handle multiple phone lines on in an inpatient care setting, making sure these calls are forwarded to correct staff if needed.
• Communicate with Veterans, employees, interdisciplinary staff, and external agencies to ensure clear instructions and smooth coordination.
• Support clinical teams by preparing and verifying lab requests, imaging orders, exposure follow-up requirements, and testing compliance.
• Coordinate and schedule appointments across Employee Occupational Health (EOH), Compensation & Pension (C&P), and Home-Based Primary Care (HBPC), including physicals, follow-ups, C&P exams, HBPC visits, and urgent triage referrals.
• Identify scheduling gaps, operational barriers, and workflow issues and escalate or assist leadership in implementing corrective actions.
• Update demographics, enrollment information, PCMM assignments, and personnel records with complete accuracy.
• Maintain tracking logs and reporting systems for C&P exam status, HBPC census and metrics, EOH exposures, OSHA reports, TB conversions, VERA monitoring, and timeliness indicators.

Work Schedule: Monday-Friday 8:00AM-4:30PM
