Mental Health RN - ACT Case Management Specialist

Horizon House, Inc. in Claymont, Delaware is seeking a qualified Registered Nurse (RN) to join our ACT Case Management Hope team. The role involves coordinating behavioral and physical health care needs, developing recovery plans with participants and staff, and managing a caseload with timely documentation.
