Associate Medical Officer

JOB OVERVIEW

The Primary Care Physician (PCP) is a licensed, Board‑certified or Board‑eligible professional in internal or family medicine who plays a key role as part of the clinical operations team. The PCP provides direct patient care and assessments primarily in ambulatory health‑care settings, but may also serve in acute care, nursing homes, skilled nursing facilities (SNF), and home settings. Responsibilities include geriatric assessment, medical history, physical exam, diagnosis and treatment, development of the plan of care, health education, specialty referrals, case‑management referrals, follow‑up, and clear documentation according to ChenMed standards for quality, service, productivity, and teamwork. The PCP also participates in clinical rounds and conferences and documents progress notes and summaries in depth.

RESPONSIBILITIES

Function independently as a primary care practitioner as part of a patient‑care team. Independently assess acute and non‑acute clinical problems. Perform and document physical assessments and patient histories; analyze trends, develop, document, and implement patient‑management plans, provide counseling and education. Plan patient care based on in‑depth knowledge of the specific patient population and/or protocol, anticipating physiological and psychological problems, considering cultural background, level of understanding, personality and support systems; serve as patient advocate. Manage patients by writing orders for admissions, transfers, discharges; ordering and interpreting laboratory and diagnostic studies; ordering medication and treatments; consulting specialties such as dermatology, neurology, ophthalmology, endocrinology, surgery, intensive care, infectious disease, hematology, psychiatry, social service, dietetics, etc.; documenting in progress notes and summaries. Participate in patient‑care rounds and conferences; communicate management strategies to team members. Collate with multidisciplinary team members to ensure patient management strategies meet patient‑care needs. Recognize situations requiring immediate attention and initiate life‑saving procedures when necessary. Use advanced communication skills to problem‑solve complex situations and improve processes and service to patients. Collaborate with multidisciplinary team members to analyze and evaluate current health‑care delivery systems; identify and implement new practice patterns. Participate in outside activities that enhance personal and professional growth and development. Initiate arrangements, write orders for discharges, and complete appropriate paperwork. Work collaboratively with physicians, nurses, PT, social workers, family and key caregivers to transition patients to lower levels of care as medically appropriate. Advocate for patient education and ensure needed services and education are provided. Introduce self to patient/family; explain primary‑care provider role. Facilitate patient/family conferences to review treatment goals, optimize resource utilization, provide education, and identify needs. Enhance collaborative relationships to maximize patient/family ability to make informed decisions about goals of care, palliative care, and hospice. Manage utilization and financial aspects, including reimbursement for services. Facilitate discharge to appropriate level of care and use preferred providers when additional services are required.

QUALIFICATIONS & EXPERIENCE

MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty. Current, active MD licensure in the state of employment. Minimum of 1 year of clinical experience in geriatric, adult, or family practice setting; preferred if Lead PCP is a ChenMed PCP Partner. Completion of ChenMed training, including Masterful Conversations and meeting facilitation as part of development plan. Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty is preferred; Board eligibility is required. Maintain board certification in terminal specialty through MOC, CME, or requisite exams. Current DEA number for schedule II–V controlled substances. Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within first 90 days.

PAY RANGE

238,832 – 341,189 Salary (base hourly rate or base annual full‑time salary). Final compensation will vary based on experience, education, geographic location, and other relevant factors. This position may also be eligible for bonuses or commissions. #J-18808-Ljbffr ChenMed
