RN Care Manager, Complex Care

Position Summary As an integral member of the Project SHINE clinical care management team, the Registered Nurse (RN) Care Manager will have the opportunity to make a profound impact on the lives of people living with HIV/AIDS and people at risk for HIV transmission. The role requires flexibility, the ability to work with high‑need communities and may vary day‑to‑day to meet members where they are. Outreach methods may vary based on organizational needs and may include telephonic or in‑person interactions. Schedule Mon, Tues, Wed, & Fri 8:00 AM - 12:00 PM, & Thurs 1:00 PM - 5:00 PM with flexibility based on departmental needs. Essential Duties & Responsibilities Coordinate treatment of all STI consults, results, and management (Syphilis, Gonorrhea, chlamydia, etc.). Evaluate and follow up on all patients receiving treatment for HIV, PrEP, and Npep. Provide specialized nursing support for medical providers, interpreting STI lab results and assisting with HIV diagnosis and linkage to care. Complete nursing intakes by assessing patients, obtaining histories, and performing appropriate tests and observations. Provide comprehensive care, addressing care gaps, vaccinations, and annual screenings. Develop individualized patient care plans, including adherence and outreach strategies, in collaboration with the Project Shine team. Assume a leadership role on the multidisciplinary care team. Interact with patients and staff in a professional and respectful manner and maintain a positive attitude. Safely administer medications, tests, and treatments as ordered and manage medication refills with provider authorization. Document patient histories, assessments, plans, and treatments thoroughly and concisely. Implement Domestic Violence and Abuse Screening policy. Follow established departmental policies for environmental safety and infection control. Participate in quality improvement related to nursing functions. Attend required departmental activities and meetings. Perform other duties as assigned. Education High school diploma required. Completion of vocational/technical training beyond high school in an accredited nursing program required. Licensure as a Massachusetts R.N. BCLS certification (per departmental policy) required. Experience New graduates are welcome. Two years experience in HIV nursing preferred, preferably in a community health setting. Skills & Abilities Spanish bilingual/bicultural skills preferred.
