Strong experience in discharge planning is essential. The role requires collaboration with interdisciplinary teams to coordinate care effectively. A minimum of two years of experience in a relevant acute hospital setting is mandatory. Candidates must demonstrate proficiency in patient evaluations and care planning. Knowledge of psychosocial assessments and crisis intervention techniques is crucial. The ability to coordinate services post-discharge is necessary. Familiarity with charting systems is required. Experience with diverse patient populations, including adults and geriatrics, is preferred. Candidates must be prepared to engage in daily interdisciplinary rounds and case conferences. This position requires strong communication skills and the ability to work effectively within a team.
