Key Responsibilities Conduct admissions, skilled nursing visits, and ongoing patient assessments Develop and manage individualized care plans in collaboration with physicians and clinical teams Provide hands‑on patient care, education, and disease management support Monitor patient progress and adjust care plans as needed Coordinate care with therapists, home health aides, and other healthcare professionals Maintain accurate and timely clinical documentation Support patients and families with compassionate, holistic care Qualifications Associate or Bachelor’s degree in Nursing Active and unrestricted Registered Nurse (RN) license Current CPR certification Minimum one year of nursing experience preferred Strong background in Med Surg, ICU, ER, or acute care preferred Home Health experience is a plus Excellent communication, clinical assessment, and patient care skills Why Join Us?
