Job Summary:

Provides outpatient physical therapy services to adult patients in home health and facility settings, promoting recovery and functional independence through personalized treatment plans.

Responsibilities:

• 
  Evaluate patients' physical conditions and develop tailored therapy plans.
  
  
• 
  Administer therapeutic exercises, manual therapy, and modalities to improve patient outcomes.
  
  
• 
  Document patient progress and update treatment plans in compliance with EMR systems.
  
  
• 
  Travel to patient homes and facilities, covering up to 75 miles from the clinic as needed.
  
  
• 
  Communicate with care teams to coordinate care and ensure continuity of services.
  
  
• 
  Adhere to all safety, licensing, and documentation protocols, including timely completion of notes.
  
  

Qualifications:

• 
  Hold an active licensed physical therapist (PT) credential.
  
  
• 
  Minimum of 1 year of recent adult home health and/or hospice experience.
  
  
• 
  Proficient in HomeCare HomeBase, Oasis, and Outlook EMR systems.
  
  
• 
  Demonstrate proficiency in OASIS and use of EMR within the last 6 months.
  
  
• 
  Valid drivers license, reliable transportation, and auto insurance required.
  
  
• 
  Ability to travel up to 75 miles, with flexibility for varied schedules, including 4x10 Baylor shifts or standard M-F hours.
