Patient Care CoordinatorAt All American Home Care, the Patient Care Coordinator ensures clients receive consistent, reliable home care services while maintaining compliance and operational accuracy. This role serves as the central point of coordination between clients and caregivers. You help ensure schedules are covered, documentation is accurate, supporting daily scheduling, coordination, and communication.
