Callback, call, overtime, travel, and holiday pay are included in the flat rate there is only one rate that can be billed. All expenses must be built into the hourly or shift rate , depending on the assignment. No separate billing for travel, overtime, holidays, or other add-ons. Credentialing Timeframe (with clean background/MP/criminal history): AZ Licensed : ~6 weeks IMLC : ~2 months Not Licensed in AZ : ~3 months Credentialing can begin while license is in process. Submission Restriction : Agency ownership is 1 year from date of submission; providers cannot be presented by another agency for one year after submission.
