Hospice Nurse Practitioner

Gentiva is seeking a Hospice Nurse Practitioner to deliver compassionate, home-based end-of-life care. In this role, you will conduct home visits, perform comprehensive assessments, manage pain and complex symptoms, and develop individualized care plans that honor patient goals and dignity. You’ll collaborate closely with physicians, nurses, social workers, chaplains, and caregivers, providing education and emotional support to families.
