LTSS Service Care ManagerImmediate need for a talented LTSS Service Care Manager. This is a 12+ Months Contract opportunity with long-term potential and is located in New Port Richey, FL, Port Richey, FL, Hudson, FL, or Land O' Lakes, FL (Hybrid). Please review the job description below and contact me ASAP if you are interested.Job ID:26-32446 Pay Range: $27 - $29/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).Key Responsibilities:Manage a caseload for healthcare members with long-term care needs.Conduct monthly and quarterly member contacts, with approximately 80% travel.Perform assessments with members, caregivers, or providers to gather essential information for care plan development.Monitor service delivery and follow up with members and caregivers through in-person visits and telephonic contact.Authorize and coordinate referrals for services, ensuring no gaps in provider services.Collaborate with discharge planners, physicians, and other stakeholders to create effective discharge and care plans.Assist members in filing and resolving complaints and appeals.Key Requirements and Technology Experience:2+ years of experience in care management, specifically in long-term care.Proficiency with electronic medical health records and Microsoft Office.A valid driver's license is required for travel.A Bachelor's degree in a relevant field.Experience in home visits and familiarity with care management tools.Our client is a leading Healthcare Industry and we are currently interviewing to fill this and other similar contract positions.
