Responsibilities Administer medications and treatments as prescribed by the physician in accordance with Simpson’s medication administration policy. Monitor and record vital signs such as blood pressure, pulse, and respiration rate. Assist with activities of daily living, including bathing, dressing, grooming, and meal preparation and feeding as needed. Provide wound care, including cleaning and dressing wounds, and give in‑dwelling catheter care occasionally when needed. Assist with mobility, transfers, and positioning of residents. Document all care provided and maintain accurate resident records. Communicate effectively with residents, families, and other healthcare professionals. Adhere to all policies and procedures related to resident care and safety. Assist in creating, updating, and maintaining resident care plans as necessary. Participate in continuing education and training to maintain licensure and stay current with nursing practices through Relias on‑line training as required under PA regulations on an annual basis. Qualifications HS Diploma or equivalent (e.g., GED); if the diploma is from a school outside the US, it must be translated to English. Successful completion of an accredited Licensed Practical Nurse training program. Current Pennsylvania LPN License with no judgments or disciplinary action. CPR Certified, First Aid Certified; AED Certified (Preferred). 1 year experience working as an LPN in long‑term care; experience in a memory care facility is preferred. Knowledge of nursing practices and procedures. Effective communication with residents, families, and healthcare providers. Ability to work as a team member and follow directions.
