Physical Therapist ($15K RELOCATION BONUS)

FOX Rehabilitation is seeking a passionate and clinically driven Physical Therapist (PT) to join our clinician-owned, geriatric-focused private practice. In this role, you will provide evidence-based, patient-centered rehabilitation services to older adults across a variety of care settings, helping them maintain safety, independence, and improved quality of life. As a Physical Therapist with FOX Rehabilitation, you will perform comprehensive evaluations of adult and geriatric patients, identify functional limitations, and design individualized treatment plans grounded in current best evidence. You will implement a full spectrum of therapeutic interventions, including therapeutic exercise, gait and balance training, functional mobility training, fall-prevention strategies, and manual therapy as appropriate. You will document all care accurately and efficiently using electronic medical records, track patient progress through objective outcome measures, and adjust plans of care to achieve meaningful, patient-driven goals.

You will collaborate closely with an interdisciplinary team that may include physicians, nurses, occupational therapists, speech-language pathologists, social workers, and care coordinators to ensure a holistic and coordinated approach to care. Patient and caregiver education will be a key component of your work, empowering families with strategies to support safe mobility, manage chronic conditions, and promote long-term functional success at home and in the community.

This is a full-time, field-based role in which you will travel to patients in their residences or community-based settings to deliver one-on-one, functional, and goal-oriented physical therapy.
