As a Care Manager, you will provide in-home support and companionship to individuals in our community while helping them live safely and comfortably in their own homes. Responsibilities may include:

• Assisting clients with Activities of Daily Living (ADLs), including: Bathing, Dressing, Grooming, Toileting, Mobility assistance, Meal preparation, Medication reminders, Light housekeeping and laundry
• Providing companionship and emotional support
• Assisting with errands and community activities
• Maintaining accurate documentation and communication with the care team
• Creating meaningful connections with clients and families through compassionate care

Travel Requirements: This position requires travel between client homes throughout the shift. Care Managers must have: A valid driver's license, Reliable transportation, Current auto insurance, The ability to safely travel to multiple client locations as assigned.
